petitions

i am doing a big pertitions project but im not sure what i have to put on the pertition paper. i know i need:

pertition reason
name
reason?
signature?

but do i need to aask for:

phone number
address

and is there anything else that i should include / ask for?

well if im signing a petition i dont mind putting my address – or just my postcode to give general area, I dont like leaving the whole address, am funny bout that sort of thing – and if it insists on phone number i dont sign it because quite frankly i dont want to give my number out – so yup, maybe ask for it but dont insist on it.

i was thinking of making the address and telephone optional but i wasnt sure if they would have to cheque the names so would require an address or number

i would always feel happier leaving my email rather than my address or phone number
